2. Tick
in the box next to the items you require - this can be handed in,
faxed to 0113 230 4219, or posted. Please do not telephone to request
a repeat prescription.
3. Your
prescription will be ready to collect from the surgery within 48 hours.
If you require your prescription posted back, please enclose a SAE.
4. Periodically
the doctor needs to review your medication with you so you may be
asked to make an appointment before you can receive your next prescription.
5. If
you want a prescription that is not on the list, please complete one
of our repeat prescription forms. (The doctor may wish to see you
before prescribing)
6. If you require
the prescription to be collected by a chemist, please make it obvious
on the prescription which chemist and which branch
7. Since
we occasionally have queries about your medication, please can you
make sure we have an up-to-date telephone number so that we can contact
you,
