Appointment information

How to book
If you would like to speak to a social prescriber, please ask a member of the practice to be referred. You can send an online “admin” request asking to be referred to the team.
We will contact the Social Prescribing team and they will get in touch to book you in for an appointment. They aim to contact you within a week.
