Register as a new Patient

You can register with us as a new patient if you live in our inner practice area.

If you have been registered in our inner practice area and will be moving out of our practice boundary, you may remain on our list, however you must live in Leeds and be aware you will not receive home visits. Please see the map below to check if you are in our catchment area.

How to Register

Visit the surgery in person and complete the application forms at reception or hand in a copy (forms below)

If you have children under the age of 5, please ensure the Health Visitor Form is filled out, this is located at the end of the children’s registration form.

Register for Online Services

If you would like to register for an online account (NHS app, Patient Access), please fill the following form and email it to wyicb-leeds.practiceadministration@nhs.net with a copy of ID.

Updating Your Details

Please let us know if you move address, change your phone number(s) or email. You can do this in person or if you are registered for online services, you can do this via the patient portal. You can also let us know via our e-consultation service here.
